What is
What is melasma?
Melasma is a chronic pathology consisting of facial hyperpigmentation (especially on the cheeks, forehead or upper lip), of multifactorial origin, that requires a personalized medical approach.
Therefore, all treatments are performed after an individualized medical assessment, accompanied by strict photoprotection and home treatment, which are essential to maintain the results in the long term.
There is no single treatment valid for all cases, so in
we work with combined protocols that act on the origin of the pigment, the
skin inflammation and the overall quality of the skin, minimizing the risk of relapse.

Causes of melasma
Melasma is caused by an overproduction of melanin in the melanocytes, activated by different factors:
- Solar radiation (UV and visible light).
- Hormonal changes (pregnancy, contraceptives, menopause).
- Genetics and phototype.
- Oxidative stress and inflammation.
- Photosensitizing cosmetics or medications.

The first step in the treatment of melasma is a dermatological diagnosis. At Clínica Tufet we perform a complete medical evaluation to identify the cause and plan the most appropriate protocol. In this way, our dermatologists and aesthetic doctors determine the depth of the pigment (epidermal, dermal or mixed) before starting treatment.
Diagnosis of melasma is based on clinical examination, Wood’s light and digital dermoscopy. In some cases, a skin biopsy may be required to rule out other pigmented lesions.
Treatment options
There is no definitive cure for melasma, but at Clínica Tufet we achieve very significant and stable improvements through a comprehensive medical and dermocosmetic approach:
1. Rebalacing Protocol (Renophase®)
The rebalancing protocol combines progressive peels and advanced medical cosmetics with Renophase® tranexamic acid, phytic acid and niacinamide. 2. Renophase + PicoWay® Protocol
Recommended in patients with:
- Resistant or recurrent melasma
- Deep or long-standing melasma
- Lack of response to previous treatments
- Stains associated with sun damage
- Need for more intense and long-lasting results
- 4 sessions of Renophase medical peeling
- Depigmenting home treatment
- 2 PicoWay® 1064 nm laser sessions
- Possibility of combining with IPL according to medical assessment.
3. PicoWay® Protocol
Protocol especially indicated in patients with:
- Deep or persistent melasma
- Stains resistant to other treatments
- Good condition of the skin barrier

Resistant or recurrent melasma
In some cases, melasma may recur or show resistance to conventional treatments.
